It is Industry standard that The Model Release form, means the model is actually "Releasing" her rights to the images over the rights of the Owner ; that is The Photographer.
The photographer is actually the one who owns the images!!
Although in some cases the model may be given rights to publish images she has posed and paid for. For example, posting these on Adultwork, social media etc.

What do you do with the images you have taken?
Do you ever publish them?
How are they stored?
Where are they stored and for how long?

It is industry standard that the photographer owns the images, except sometimes in the case where he is in the employment of say a newspaper, and he is taking photographs as an employee of that publication.

Can you please tell us a little more about your procedures?

I use the term Model Release Form, that's probably not the right wording. In terms of the law, me (as the photograph) own the copyright, but my MRF signs the copyright over to the model I'm working with. That way, she (or he) owns the copyright and can use the images as they wish. I only get to post images with their permission, either via a PM, email or text, but something stating 'Yes you may use that image'. 

I have worked with several girls who's images I would have loved on my portfolio, but they have said no, so I haven't used them. 

They are stored on external HDDs that I have. The only time they are on line is on a secure website (with password protection) that is only shared with the model. I usually keep them on there for good. 

I like to go through everything with the model on before hand, to explain everything. On the day of the shoot, the first thing I do is sit down with whoever I might be working with and go through the MRF (I'm going to have to change the name of that) and sign it with her (or him), on there is clearly states that by signing the model has full copyright over the images and I cannot use them without their permission, but the model can use them at her discretion. 

I hope that's made sense. I always consider the images I take as belonging to the model I'm working with, so she doesn't have to worry about where they might end up, since I only use ones with permission. I'm happy to send people a copy of the MRF so they can read it first.
