I honestly don't think that escorts eyeball guys before booking in an attempt to see what they are like or if they fancy them. What they are doing, is making sure the guy is not drunk, making sure he's not a guy that they do not want to see because of a previous booking that went badly, turns out to be someone they know, has other guys with him and is looking to theive and/or hurt/ rape her, has a mask on or is holding one and is potentially dangerous, is actually there and isn't one of these guys that just pretend in order to get the girls address. (Stalker alert)
Spy holes are practically useless. If a bunch of guys are looking to cause harm to, or steal from a girl, then one guy will stand in front of the door and the others against the walls at the side. Safety chains can be broken, and if they have a knife then they can still reach in and hold it to your throat.
As for punters safety. You need to put your own safety procedures in place. One of them could be to only see established escorts with plenty of feedback, twitter profiles to see that they are a real person, keep an eye out for reports of scams and robberies.
I know that seeing escorts can be dangerous for the clients, but no where near as dangerous as it is for the escorts.
