I share your concerns Louise, it will never stop unscrupulous website owners that deliberately go for domain names that are easily misspelled and are very similar to something popular.
I wouldn't ordinarily condone something being mandatory (it can be very nanny state etc) but I think if adult oriented sites were only allowed .xxx domains there could be no ambiguity. If you are heading for a .xxx domain you know what you are going to be getting.
Even the most inept parent would be able to see how to easily block a .xxx website too.
My concerns though are that a whole portion of the net could be censored by disreputable governments.
The whole subject is full of pro's and cons
